The issue of patients undergoing a second heart operation poses a great risk of mortality. Surgical cardiac procedures have a significant high morbidity risk after conduction of the first operation. The scientific article used for this paper examines factors that could prove useful for the development of a scorecard, which could calculate the amount of risk involved for a patient who would undergo a second cardiac operation. Development of the scorecard considers a variety of factors that contribute to the risk involved. Medical practitioners would find the card useful in the elimination of cases of mortality by making optimal decisions for patients who have to undergo a second heart operation.
The article for the study is ‘Predicting In-Hospital Mortality After Redo Cardiac Operations: Development of a Preoperative Scorecard.’ The sample studied participants who had all been to the same clinical center for a redo surgical operation for both initial and follow up (Launcelott, Ouzounian, Buth & Légaré, 2012). The criteria for selection of participants were systematic. The system involved peer review procedures with cardiac specialists on a weekly basis. The sample included 1,521 patients throughout fifteen years. The study was prospective and conducted with the anonymity of intricate details. Qualitative methods proved useful for data acquisition. The team obtained data from the registry of the health facility, whose database contained data on demography, operative variables, post-operational outcomes, and comorbidities. The instruments used for the data collection were an observation of a focus group comprised of the study participants, and document analysis of material obtained from the database. Conduction of the research involved statistical analysis of the available documents (Launcelott, Ouzounian, Buth & Légaré, 2012). The process included a separate examination of the medical characteristics of participants who survived and who died after the operation. Reporting of continuous variables with normal distributions was as standard deviation and mean, and comparison of the values was through a two-tailed t-test. Interquartile range and media served the reporting of continuous variables that did not follow a normal distribution, and comparison of the two values was through a Wilcoxon rank sum analysis. The team used the Fisher’s exact test and analysis by x² to report categorical variables as either frequencies or percentages. Multivariate analysis served for the analysis of candidate variables, and linear regression analysis for the assessment of multicollinearity. The prediction of mortality would be with a regression model to check for parsimonious logistics.
