According to Boehning and Punsalan (2023), the four Advanced Practice Nurse (APN) roles that are recognized in all United States (U.S.) territories and states are certified nurse practitioner (CNP), clinical nurse specialist (CNS), certified nurse-midwife (CNM), and certified registered nurse anesthetist (CRNA). APNs in Illinois face barriers related to the need for newly graduated APNs to practice under the mentorship or supervision of an established clinician, making the transition to optimal practice thresholds more challenging. Illinois is a restricted or reduce practice state with enormous practice transition requirements. Federal regulations, including organizational and institutional, as well as the Centers for Medicare and Medicaid Services (CMS), undermine APN-provide services and care. Organizational and institutional barriers include regulations that constrain APN authority and roles, including provider credentialing issues, the absence of healthcare facility privileges, and issues related to reimbursement (Kleinpell et al., 2023). These barriers interfere with APNs' ability to offer optimal care follow-up, constrain patients' ability to choose their preferred provider, and reduce APNs' care visibility.
Different Forms of Competition in the APN Practice Environment
APNs practicing under Illinois Registered Nurse License must consult physicians when prescribing benzodiazepines or Schedule II narcotic medications, including opioids. This creates competition between APNs and physicians because of supervision requirements that restrict APN's capacity to offer primary care services within their educational attainment and practice capabilities (Legal Information Institute, n.d.). At the national level, APNs' autonomous practice requirements conflict with those of physicians and physician assistants, thus creating unhealthy collaborative practice archetypes that reduce healthcare professionals' productivity. Not allowing APNs to practice to the full degree of their education and licensure creates an unhealthy relationship and rivalry between diverse healthcare professionals, undermining patients' ability to access high-quality and efficient care (Kleinpell et al., 2023).
