     In forty to sixty year old adults, (P) with type II diabetes and A1C of eight percent or higher, (I) does integration of supervised home visits by nursing students, (C ) compare to traditional care, (O) decrease A1C, reduce cost, and increase compliance moving patients to self-care, (T) over a three month period?  Diabetes is a chronic disease that requires continual monitoring and intervention to prevent complication. Patients must learn how to evaluate themselves, decide what actions to take to attend to their needs, and perform those actions (Orem, 1985). Glycemic control is the goal to implementation of home visits. Designing effective interventions to help each patient reach their goal will be the focus.

In the United States, Type 2 diabetes mellitus has been ranked sixth leading cause of death and is directly associated with over 73,000 deaths and as a contributing factor in over 220,000 deaths. In America, there are over 20 million diabetes cases. Management of type 2 diabetes aims at maintaining the level of glucose in blood at close to normal (below 5.7) as possible. Patients’ self care is encouraged as a means of lowering mortality death and there are several theories that encourage self-care (Ripsin et al, 2009).
Orem’s Self-Care Theory involves three concepts namely self-care deficit, self-care and nursing systems. It argues that all clients have the potential and are willing to care for themselves as a means of enhancing their rehabilitation. This attitude is influenced by life-stage-specific needs, developmental stages as well as universal self-care needs such as food, water, air, elimination, solitude/ interaction, activity/rest, promotion of normality and hazard prevention. However some clients present with self-care deficit hence intervention by a nurse is required. Orem’s theory utilizes three scales namely; Denyes self-care agency scale, Hanson and Bickel’s self-care perception agency scale and Fleisher and Kearney’s exercise of self –care agency scale (Raingruber, n. d).
