Community vs. Population  Community health is also concerned with factors that affect the health of people at the group and family level, while community health focuses on determinants of health influencing populations, including health patterns and behavior of people. Community health involves delivering health services to families, groups as well as individuals, while public health is concerned with evaluating the health needs of a population. In community health promotion and disease prevention takes into account how different health outcomes affect health outcomes. On the other hand, population health Population health also focuses on specific community and geographic groups, while highlighting similarity in health behavior affects health outcomes (Noble, & Casalino, 2013). Community health nurses play a crucial role in promoting health outside hospital settings and home as they focus on the population health concerns, in delivering care and conducing health surveillance Population Problem  Nutrition, physical activity, and obesity are health indicators of the-well being of this population. Promoting health through good nutrition, maintaining adequate body weight and adequate physical activities to improve the health outcomes and quality of life (Healthy People, 2020) There is concern than more than a third of the targeted population is at risk of serious health conditions because of poor nutrition, obesity and lack of physical activities. Promoting healthy lifestyle behaviors reduces obesity levels and improves nutrition. Population-based initiative raises awareness about obesity prevention initiatives, including nutrition and physical activities (Raine, et al., 2010). This makes it easier to strategy the elderly while engaging with various stakeholders to coordinate health initiatives. Ensuring inclusivity of population-based strategies allow the community health nurses to undertake the monitoring and surveillance of the elderly people at risk of poor nutrition and obesity. Community health nurses promote health among the elderly by influencing them to improve their lifestyle, and may alter the food environment and increase the intensity of physical activities. The ethics of beneficence highlight the case for outlining the benefits of interventions, while ensuring that the targeted population understands the importance of meeting the dietary guidelines.
