* A multicenter, double-blind placebo-controlled clinical trial was used.
* Involved 408 randomized patients.
* Randomized patients with HbA1c of 7.0%, 140mg/dl Fasting Plasma Glucose, and 1 ng/ml C-peptide
* The patients received a placebo or 7.5, 15, 30, or 45 mg of Pioglitazone (o.d) for 26 weeks.
* Significant mean decrease for patients receiving 15, 30, or 45 mg Pioglitazone. HbA1c range of 1.00 to 1.60% and FPG 39.1 to 65.3 mg/dl difference from placebo.
* FPG decrease early in the second week.
* Maximum FPG decrease was after 10-14 weeks and continued to the end of therapy.
* There was significant decrease in mean percent triglycerides, increase in HDL cholesterol, and small changes in total cholesterol and LDL for patients receiving 15, 30, or 45 mg Pioglitazone.
* Patients being treated for the first time had greater improvements in HbA1c and FPG. Placebo difference was 2.55% and 79.9mg/dl for those receiving 45-mg compared with those who have been treated previously treated.
* Adverse effects for Pioglitazone and the placebo were the same.
* There was no drug-induced hepatotoxicity or elevations of alanine aminotransferase levels.
