Introduction
In the clinical practice, there are clinical problems which arise in different patients. In the ICU, the clinical problems are a bit more complicated due to the sensitivity of the condition of those patients. The clinical problems require proper knowledge from the nurses as well as the other health care providers so that they can give the correct health care service required. This paper analyses the clinical problem of delirium in ICU patients, in view of the characteristics of this problem that qualify it as a clinical problem as well as the description of this issue.
Delirium in ICU patients
Delirium in the ICU patients denotes the disturbance of consciousness of the patients in the ICU. This is usually accompanied by change in their cognition. Delirium in the ICU patients mainly occurs with an acute onset but takes a fluctuating course. Svenningsen, Tonnesen, Videbech et al (2014) note that; the most commonly attacked patients are the mechanically ventilated patients in the ICU. It is difficult to diagnose delirium unless the specific tools for its diagnosis such as CAM-ICU are used. In order to ensure the accuracy of the diagnosis, it is crucial that the Cam- ICU is used on the patient by a physician with the knowledge and skill of its utilization. There are three known types of delirium are the hyperactive, hypoactive and the mixed type.
Delirium in ICU patients is a clinical problem because it is an issue of the patient which calls for the attention of the physician in order to diagnose it and come up with possible solutions which can be used to mitigate the effects and ultimately heal the problem.
Rivosecchi, Smithburger, Svec et al (2015) acknowledge that; delirium in ICU patients is a clinical issue in the sense that its diagnoses and strategy for solving it is very much aligned to the perceived difficulty of the issue. In this sense, the diagnosis and the measures taken to solve the problem highly depend with the complication of the delirium. This calls for consideration of the type of delirium for instance, hyperactive or hypoactive. It is also important to consider the uniqueness of the patient for instance, young patient or old. The conditions in the ICU are also considered, for instance, the mechanically ventilated patients are more likely to get attacks of delirium.
Delirium in ICU patients requires diagnostic reasoning in clinical practices like any other clinical problem. Employment of psychological processes in order to come up with the identification and solution to the clinical problem is a common characteristic of clinical issues which is evident in dealing with delirium in the ICU patients.
