1.Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and change. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.
2.Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics, and medical homes.
3. In 800-1,000 words summarize the feedback shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform.
A minimum of three scholarly references are required for this assignment.

It’s safe to say that nurses play a significant role in the betterment and prosperity of the society. They provide timely treatment to the patients and help them improve their lifestyles to an extent. Unlike doctors, nurses are the primary caregivers in any healthcare center. It is important for a nurse to gain more and more knowledge to grow further and to evolve with time, especially when the expectations of patients touch the peaks. She should equip herself with skills regarding how to use medical devices and how to deliver quality care to the patients. Getting a degree is not enough; a nurse needs to be aware of the technological and scientific advances, and be ready to face the challenges during her working hours. The changes regarding medical homes, nurse-managed health clinics, and the ACO (Accountable Care Organization) need to be taken into account while providing healthcare facilities to different patients (Soffer, 2014).
In the continuity of care, the goal of a nurse is to provide affordable and quality facilities and healthcare services to all patients, specifically those who are suffering from cancer, HIV/AIDS, and other chronic diseases. When it comes to providing community-based services, a nurse has to visit her patients regularly in order to make them feel comfortable and relaxed in their own residences. She should bring with herself all medications and equipment that might be needed during her visit. In case she cannot visit patients personally at their homes, she can provide assistance through the internet. The field of telemedicine is quite extensive and well-versed. Nurses can now interact and communicate with their patients through different web-based platforms and can provide free consultancy services to people living in rural areas. Nurses can implement different disease management systems to help patients recover from minor to severe diseases at a fast speed (Trupkovic et al., 2017).
Nurses can make the wise use of different health information systems, which are meant to provide quality services to patients at affordable costs. With these systems, nurses can easily access important information about the patients such as their medical history, age, family background and other similar things that might help be of help. The ultimate goal of the Accountable Care Organization, nurse-managed healthcare clinics, and medical homes are to help patients improve the quality of their life. All types of diseases are to be cured while reducing the care expenditures so that more and more patients can benefit from these systems (Evio & Bonito, 2017, p. 70). If the care being provided to them is costly, then many poor and needy patients may skip to other options or may not think of visiting doctors and nurses for medical assistance. On the other hand, if care being provided to them is affordable and reachable, then a lot of patients from both rural and urban areas can benefit, and the rate of deaths can be reduced to an extent.
