In today's society, obesity has become one of the leading preventable causes of death. It can be largely accounted for by poor nutrition and lack of exercise. As a nurse, it is important to understand how nutrition effects the body's different systems and how it can influence a person's psychological state.
Read the following case study on nutrition assessment. While reading, reflect on your current knowledge of nutrition and your role as a nurse in teaching good nutrition.
Case Study:
M.M. is brought to the emergency department by family members. The patient is 74, fairly alert, and oriented. He states that he just vomited a large quantity of frank blood. History includes complaints of decreased appetite, back pain, and recent weight loss. He has a history of fractured vertebrae 15 years ago and use of two packs of cigarettes a day for 20 years. He was a welder by occupation. History also includes colon resection 18 months ago for cancer.
Based on the case study, respond to the following: 
•Why is a nutritional assessment important for this patient? 
•What additional information may be needed before a thorough nursing assessment can be made? 
•What medications may be causing gastrointestinal side effects?

Nutrition is a very central health aspect of the elderly populace as it affects the aging process. Recent years have recorded an increase of prevalence of malnutrition in these populations. The process is associated with a decline in major body functions such as impaired muscle functions, immune dysfunctions, decrease in body and bone mass, anemia, poor wound healing, reduced cognitive functions, delayed recovery from surgery, high hospital readmission among many others. Reduced appetite accompanied with falling biological and psychological functions are evident (Dent et al, 2014).
MM case is not different; he is a senior citizen aged 74 with a complicated health history, which necessitates a nutritional assessment. The evaluation is important to both the patient and the nurse. To the patient, a dietary assessment that requires the patient to give an account of his/her intake in the last 24 hours or maybe the last one week would explain the unintentional weight loss. Another important evaluation is the clinical assessment, which indicates nutritional deficiencies. Here, the medical history of the patient is examined to establish the right diagnosis for the patient and to ensure that previous ailment medications are factored into the equation so as avoid adverse responses.
