This implies that the mission statement will be “to improve patient safety and quality care of outpatient diabetes patients,” while the vision statement will be “to ensure West Virginia is diabetes free in the long run.” As such, there is a need to introduce a timeline that would facilitate the realization of the above objectives. The start date for this plan will be Apr. 10, 2023, up to Aug. 10, 2023. The focus is on approximately four months to accomplish the plan, especially the short-term goals. The plan runs for months because it involves the acquisition of medical and technological equipment and the need to train stakeholders, including patients and healthcare practitioners, regarding the benefits of electronic health records and how to apply them to achieve positive outcomes.
Short-Term Goals
To improve patient safety and quality care of outpatient diabetes patients (1st Short-term Goal)

Since the focus on EHR includes improving the organization and exchange of patient data, one can conclude that they can receive medical advice without necessarily adopting the in-patient hospital approach, thus increasing flexibility. This is also a cost-effective technique. This goal should take approximately two weeks to 2 months to achieve.
To manage diabetes symptoms efficiently (3rd Short-term goal)
In this regard, Veinot et al. (2010) allude that EHR is adopted to enhance clinical decision-making, improve communication among healthcare stakeholders and manage large amounts of data. This implies that healthcare practitioners have access to data that can facilitate research and containment of diabetes symptoms. In other words, since it documents diagnosis and interventions, the patient’s access to information on the management of symptoms is expected to be efficient. The anticipated duration for this goal is between 1 and 3 months.
