The Community Assessment and Collaborator Mini-Paper Assignment

This is a two part assignment to be submitted together:

 

Part one: The purpose of the Community Assessment Mini-Paper is to provide a guided and open inquiry based assignment to assess a population that you encounter in your work setting in order to form the foundation for an in-depth understanding of cultural, "racial," linguistic, gender, and social diversity in modern societies. (Incorporate your clinical time as you assess your community and speak to the community stakeholders)

 

Part two: The Inventory is to help you discover resources that are available to benefit the populations you serve or assess. As you write this paper, you should consider the work you have completed so far and further assess the population you discussed in your first discussion board assignment. Also, be sure to log any clinical immersion hours that you use as you answer these questions (we would expect as many as 6-10 hours).

 

Below you will find the required elements of this assignment.

Part one: You will use the Windshield Survey on page 11.1 in Anderson & McFarlane as a guide.

  1. Discuss 2 key elements of each of the Community Core, Subsystems, and Perceptions assessment as they relate to your population. [This may help you to focus on your plans for the Capstone Paper.] 
    1. For example, you may be looking at older women residing in the Temple Terrace area of Hillsborough County. For the Core, you would have county data for demographics, etc. but will need a closer look for history, what restaurants etc. are in the area…signs of cultural groups etc.
    2. Incorporate key core data (using the relevant demographic and ethnicity data you found in Module 1) and describe the significance to the selected population using two (2) relevant, current peer reviewed references.

 

Part two: Use the template to describe the potential resources and collaborators in the community that are available to your population of interest. Chapter 11 will also help you to consider different options. You will also want to do a site visit as part of your clinical experience (for example: What education information does the agency provide? Are there agencies working directly with your case managers that you do not know about?)

  1. Using the template, describe the population you are considering in two to three sentences.
  2. Identify at least 5 community resources that you believe could be of value. Complete the table in the template. You certainly may focus on health and social services but we would like you to really personalize it to your own practice.
  3. Write two to three paragraphs as a reflection of your findings. For example:
    1. What gaps or barriers do you identify in terms of existing care/services?
    2. What are the credentials of the providers at the agencies? Volunteer? Professional?
    3. Are there ongoing sources of financial support to keep the agency in tact?
    4. Are the missions congruent with what you are able to assess?
    5. Would any of these agencies be good collaborators as you plan for your capstone project?

 

Remember, this paper should be between 5-7 pages (not including the title page and reference pages). You will not be able to cover every area of the windshield survey, just the area that is of your interest. All submissions are expected to be well written, with proper grammar and spelling. Please provide any references that you use to provide evidence as noted in the instructions. Review the rubric before submission.

NOTE: Make sure to reference your sources (including your textbook) and your personal communications (see APA manual page 179). May use pseudonyms.

Type 2 Diabetes is one of the most common chronic diseases in the world. It accounts for nearly 90 percent of all diagnosed cases of diabetes in the world. The disease is mostly common in adults and children with obesity (Thomas, 2013). It’s on the same wavelength of risk with other terminal diseases like cancer, asthma, and coronary heart disease. There is no certain cure for diabetes, and patients can only manage it to treat symptoms and prolong life. As such, type 2 diabetes is a long term condition that lives with victims for the rest of their lives from the time of diagnosis. As a matter of necessity, therefore, patients and their families need relevant information and education on how to manage the condition as a way of improving the quality of life among affected groups.
The United States is one of the few developed nations in which Type 2 Diabetes is prevalent. In fact, the U.S. leads in diagnosis cases, with over 26 million people believed to be diabetic (John Hopkins Medicine, 2008). Of this, 12.6 million are women, which is 10.8 percent of all women aged 20 years or above. This number could be higher as there are estimated 7 million undiagnosed cases, in addition to many more overweight individuals who are at risk of being diabetic. One out of every 10 American adults aged 20 years and older is diagnosed with diabetes. According to the most recent survey findings, Type 2 Diabetes is the fifth leading killer disease in the U.S., and costs the taxpayer over $58 billion annually in terms of indirect costs, resulting from manpower loss due to sick time off, disability, and death (John Hopkins Medicine, 2008).
Among the older population, Type 2 Diabetes affects 10.9 million people aged 65 years and above, which is 26.9% of the entire age group.
In terms of ethic disparity, marginalized groups are the most affected. It affects 15.7 million Non-Hispanic whites (10 percent of all non-Hispanic whites 20 years or older), and 4.9 million non-Hispanic blacks (18.9 per cent of all non-Hispanic blacks aged 20 years and above) (Center for Disease Control and Prevention, 2013).
Type 2 Diabetes is the major cause of many health complications in the U.S. statistics from 2004 shows that diabetes-related complications were the cause of 68 percent of deaths caused by coronary heart disease. It was attributed to 16 percent of deaths resulting from stroke in persons aged 65 years and above. It increases the risk for stroke 2 to four times, and increases deaths associated with heart diseases as many times (Center for Disease Control and Prevention, 2013). It was also responsible for increased cases of hypertension, kidney disease, blindness, amputations, and nervous system disease.
