In discussing the current state of the nursing units – it was observed that there was an upward trend in central line and indwelling catheter infections. In an open discussion, it was noted that four new initiatives designed to decrease the incidence of infection were recently launched simultaneously. Some of the nurse leaders commented that it was confusing for the staff that all four initiatives were rolled out simultaneously. One of the charge nurses made the comment that there was confusion regarding expectations of performance, changes in practice, and the new procedures, since the four initiatives were implemented. The topic then switched to future state of the units where there was much discussion about what were the educational needs and priorities of the staff.
