Describe how this organization is responding the Quality and Patient Safety Movement: QAPI Description and Background Description QAPI is the merger of two complementary approaches to quality, Quality Assurance (QA) and Performance Improvement (PI). Both involve seeking and using information, but they differ in key ways: - QA is a process of meeting quality standards and assuring that care reaches an acceptable level. Nursing homes typically set QA thresholds to comply with regulations. They may also create standards that go beyond regulations. QA is a reactive, retrospective effort to examine why a facility failed to meet certain standards. QA activities do improve quality, but efforts frequently end once the standard is met. - PI (also called Quality Improvement - QI) is a pro-active and continuous study of processes with the intent to prevent or decrease the likelihood of problems by identifying areas of opportunity and testing new approaches to fix underlying causes of persistent/systemic problems. PI in nursing homes aims to improve processes involved in health care delivery and resident quality of life. PI can make good quality even better. QAPI is a data-driven, proactive approach to improving the quality of life, care, and services in nursing homes. The activities of QAPI involve members at all levels of the organization to: identify opportunities for improvement; address gaps in systems or processes; develop and implement an improvement or corrective plan; and continuously monitor effectiveness of interventions. The organization responding to quality and patient safety movement by using ABAQUIS interview with residents and family members. This is done by the administrator assigned each manager as an Ambassador to five to seven residents and ask them the same question the state will ask the residents during the state survey. The Ambassadors can identify any problems, challenging residents or concern the residents may have and resolve the problems in a timely manner. Also, the organization increases the times and types of activities, residents choice of bathing, and decrease bed alarms. 2. This paper seeks to understand and explore how the organizational cultural practice of Cypress health group influence and sharp quality control and safety management as well as management of human resource is concern. It describes to what extend Quality and Safety of patient initiatives, besides initiatives of human resource management, has influenced the Cypress group`s organizational culture. Cypress Health group (CHG), is a company that offers consulting and management services to companies in post-acute, senior housing and long-term healthcare industries. The organization culture is committed to offering the highest level of life quality for every patient and resident through its individual care planning procedures (O'Connor, 2010).

QAPI comprises quality assurance and performance improvement. QA is a process through which organizations meet quality standards and making sure the provided care reaches acceptable levels. It is a retrospective and reactive effort to assess reason for failing to meet standards set by organizations as thresholds. Activities involved in QA improve quality, but end when the standards are met. PI is a continuous, proactive study of organizational processes to decrease problems through opportunity areas identification and testing of approaches to curb causes of systemic or persistent problems. QAPI is therefore a proactive and data driven approach of improving quality of care, services and life in medical facilities. This is what Cypress group employs in responding to quality and patient safety movements.
Cypress health group collects data to use in QAPI through the use of ABAQUIS interview with family members and residence. This activity is carried out by managers and administrators assigned as Ambassadors to between five and seven residence with questions that would be asked during the state survey. Through ABAQUIS interview, the Ambassadors identify issues and problems that challenge residents and get solutions in time.
