Problem
Patient information: A.D. is a 13-year-old female who is currently in junior high school. She lives in Texas, USA. She is also a Roman Catholic.
Chief complaint: A.D. was rushed to the hospital by her mother due to complaints of extreme headache which localizes at the glabella (as pointed by the patient). This is graded 8 out of 10 on a subjective pain scale. The headache is accompanied by fever (38 degrees Celsius), nausea, vomiting, stiff neck, and hallucinations. Her mother explained that before going to the hospital, the patient’s whole body started shaking, “like there are big tremors all over the body.” She then collapsed to the floor. Fortunately, her head was cushioned by their 2-layered carpet. The generalized shaking persisted up to 2 minutes. After this, the patient gained consciousness, but she had no memory of what happened. She seemed to be disoriented to the time, place, and person and asked who is her mom and her name.
History of present illness: 7 days before the consultation, A.D. had a mild fever (37 degrees Celsius) after swimming at the nearby lake where she loves diving and trying to reach the bottom of the lake with her friends. She also complained of headache, primarily near her eyebrows graded 5 out of 10 on a subjective pain scale. Since she is near her period, she disregarded this and went to sleep afterwards, hoping that the pain would go away.
The next day, A.D. was relieved. However, she was not able to eat as much as she did before because she did not enjoy the food. She thought that she could not either smell or taste the food. After school, she went to train for her dance class with her teammates. After 2 hours of training, she had a cold shower and ended up having a fever (37.5 degrees Celsius). The fever was accompanied by nausea.
5 days prior to consulting, A.D.’s fever persisted. She did not go to school and took paracetamol (500 mg) every 4 hours. After taking 3 tablets, her temperature went down to 36.1 degrees Celsius. However, she still does not have an appetite due to a poor sense of smell and taste. She only drinks milk to fill her stomach. She was absent from school for 2 days.
2 days prior to consulting, she felt weak and frequently stumbles. When she attended her dance training, A.D. tripped over and over again. So, their team leader decided that she should rest. While going home, A.D. was disturbed by looking at the traffic lights. It made her feel uncomfortable. Upon going home, A.D. went to sleep.
1 day prior to consulting, she started vomiting around 500mL of liquids 4 times that day. During this time, she almost has not eaten for 6 days. Her mother disregarded her lack of appetite because A.D. usually goes on a crash diet. However, A.D. began to have a severe headache graded 7 out of 10 on a subjective pain scale that was relieved by paracetamol (500 mg) to 4 out of 10.
The next day, she suffered from profuse sweating then she had a generalized seizure.
