Over the years of my service in the government as the Permanent Secretary in the Ministry of Health, there has been an incessant bedeviling aliment. This has been because the ministry suffers from combating maternal deaths across the nation. The ministry’s current devices and policies seem to be incapable of stemming the stubbornly high child maternal deaths during birth. Over the past decade, the child maternal mortality has been stuck at 13% against a recommended 4% by World Health Organization, which should be attained by the year 2015. As the PS in the Health Ministry, I underscore the significance of this health problem in the country that I attribute to high maternity fees charged in hospitals. These unaffordable fees deter duly poor pregnant women who alternatively opt for village midwives. These midwives are undocumented and their profession and experience is obscure to the Ministry with records indicating that most pregnant women are succumbing to undue child deliveries. Despite the risks, most of these poor pregnant women prefer the midwives due to lack of hospital fees (Loudon, 2001).
After critically evaluating the problem and consulting with the other ministry departments, I conceptualized two proposals that I proffer for consideration by the board of health. One of the proposals involves the incorporation and documentation of the midwives in to the health system. The program includes their training in child delivery and integrating them with the salaries commission. This will ensure that skilled health practitioners undertake child delivery. Alternatively, the ministry in conjunction with the relevant ministries could lower the costs of maternal fees to one that is affordable to the poor. Maternal fee in hospitals is the chief causation of the steady maternal deaths acting as deterrence to poor pregnant women (Levine, 2007).
This report evaluates the worth of the two recommendations to adopt the best alternative for implementation by the Ministry as a solution to the grave health issue if we are to attain the recommended WHO target. The cost, efficiency, time of implementation, practicability, and durability of either program is to be incisively assessed. The first program articulates the aspect of embracing the village midwives into the health sector that would require the Ministry approximately $100 million for expediting the search and amalgamation of the midwives countrywide and enrolling them into the health sector.
The program would also ensure their teaching and training for a specified amount of time, approximately 3 months, during which the midwives will be certified as medical practitioners/nurses and enrolled into the ministry’s payroll (Bergstrom & Goodburn, 2001).
