Nurse: Hello, I am delighted to meet you today, and I hope you are as well. My name is (Your Name), and I am a nurse specialist who is here to assist you with your problem. How are you doing today?
Patient: I'm doing well. Thank you for asking.
Nurse: I'm glad to hear that you are doing well. What would you like to discuss today? (giving broad opening)
Patient: I have experienced my nightmares and chills again. I thought it would be better now because of my progress. I fear that I have to start again from scratch.
Nurse: I think it's better to talk about it. I want to listen to what you are feeling (offering self). Can you tell me more about your recent experience?
Patient: So, I know that you know about my fear of the war that I had experienced when I was in the military. We were deployed to guard one of the borders of our country when an airstrike hit us. My team and I were hit directly by the airstrike, but I luckily survived with significant bruises that got me traumatized for weeks…
Nurse: Yes, I see (Offering general lead). By the way, when did this happen? (Placing the event in time or sequence).
Patient: The incident occurred 20 years ago, and it still haunts me today. Whenever I hear loud noises, the anguish and misery of the day come flooding back to me, causing me to worry and my heart to race. There are even times when my nightmares come back again.
Nurse: I see (offering general lead). Tell me what you can remember from those dreams (encouraging description of perceptions)?
Patient: Well, the only thing that I can remember is that there had been at least three instances when I dreamt again of my days in the service. I had hallucinations of bombs being detonated other soldiers, being shot. As I wake up, I feel really hot and exhausted.
Nurse: Thank you for giving much effort to recall these experiences. You seem tense (Making observations) or uncomfortable when saying this what's with that? Tell me more about what's happening now (Encouraging description of perceptions). Is there any significant event that you think is recurring in your mind?
Patient: Since of the trauma I've had, every time I recollect that moment, I feel like I'm going to have a panic attack; I'm uncomfortable admitting this because I've avoided talking about or recalling that day since then.
