Continuum of care is a concept that encompasses a coordinated structure of care that controls and monitors patients over an extended period using an elaborate collection of health services covering all levels of intensity care (Atkinson, 2017). One major drawback of many health care systems is that they are perpetually focused on providing episodic care.
