Prioritizing the issues
The public health concern is Diabetes Type 2 in the African American community. One way to improve health outcomes and reduce the risk of complications is by promoting self-care practices (Cunningham et al., 2018). Self-management education programs would enhance the quality of life. Creating and adopting community-based plans based on nursing diagnoses and with specific goals and interventions help achieve desired health outcomes, and especially when there is community partnership (Anderson & McFarlane, 2019). Public-private partnerships, using the mass media and disseminating information on diabetes prevention and management among African Americans is necessary to achieve the program’s goals. This is also beneficial to improve treatment and outcomes. Educational interventions in diabetes improve HbA1c, weight, cholesterol, blood glucose monitoring, knowledge about diabetes and healthy lifestyles and improve the quality of life.
Risk factors and goal
             Being overweight and obese are risk factors for diabetes, and obesity levels depend on the food or diet and physical activity levels. The African American community faces a challenge accessing fresh fruits and vegetables compared to the general population. There is a high concentration of cheap processed foods in locations with African American populations. Lack of access to health information and diabetes management is another concern that makes it difficult to improve health outcomes. The goal is to improve diabetes knowledge and management where the target group has access to education tools, materials, and resources. There is also the promotion of strategies to improve diabetes care.
Contributing factors and program objectives 
Improving awareness of diabetes, understanding the risk factors, prevention strategies, and diabetes management are emphasized in the program. Improving knowledge and understanding about diabetes is necessary for diabetes self-management. However, the health care providers also need to be informed on focusing on holistic care and promoting diabetes self-management. The program is also beneficial to promote changes in health care practices to improve quality care and reduce health disparities.
Evaluation 
This is planned and is carried out at different times, but mostly after the end of the program. Carrying out a planned evaluation at 3 and 6 months will provide insights on whether there has been improvement in knowledge, self-care behaviors, and health outcomes. Other aspects considered include the quality of life and overall well-being. Making positive reinforcements helps to strengthen improvements and meet the set objectives.
Actions 
Delivery of knowledge is prioritized where the sessions should be planned in small groups. These activities may include the patient's families and/or support networks.
